WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking an experienced Family Medicine/Geriatric Nurse Practitioner to join our PACE Program team at our Franciscan Healthy Living Center Lafayette in Lafayette, IN to help meet the growing demands of our outpatient program.

WHAT WE NEED:

Ideal candidate will have nursing home care, family medicine and/or geriatric experience. Candidate would thrive in a fast-paced, autonomous, self-managed environment. Provider will need to work collaboratively with our PACE Program team to coordinate and deliver the highest quality of patient care. Candidate will also have desire to develop strong ties with patients and the community in which they serve.

WHAT YOU CAN EXPECT:

• Practice Hours: Monday â“ Friday, 8:00 a.m. â“ 5:00 p.m.

• Flexible provider schedule

• Average outpatient volume: 6 - 8 per day

• On-site training required in Indianapolis for up to 6 weeks; Mileage will be reimbursed

• Daily Duties:

  • Day-to-day care of patients

  • Review medical histories and medical assessments

  • Implement plan of care with patient and family

• Outreach potential with our Indianapolis and Dyer locations

• Will work with skilled and long-term patients
