The Home Based Primary Care (HBPC) program provides comprehensive, interdisciplinary primary care services in the homes of Veterans living within the HBPC program service area with complex and chronic, disabling diseases. The program targets frail, chronically ill Veterans requiring care by an interdisciplinary health care team to maximize the Veterans' function and quality of life. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
