MD Licensed in State of Tennessee as MD or DO; AND
Successful completion of an ACGME or AOA accredited residency program in Internal Medicine; AND
Current certification in or active participation in the examination process leading to certification in Internal Medicine by the American Board of Internal Medicine or the American Osteopathic Board of Internal Medicine; OR
MD will prove competence through the credentialing process; AND
Current competence as evidenced by active clinical practice, and documentation of performance of a cross section of the procedures / medical management of procedures being requested; AND
MD possess a current DEA Certificate of Registration with TN address ; AND
Have evidence of current professional liability insurance showing the limits of coverage and expiration date (limits should be equal to the coverage carried by the hospital–$1,000,000/$3,000,000); AND
MD should have documentation of relevant continuing medical education.
