Primary Care Physician

The Board Certified Primary Care Physician (Internal, Family or Sports Medicine) will coordinate care and provide medical services for members of SHIFT, an integrated health facility located in Chicago’s River North neighborhood. Looking for a dynamic physician, interested in building a growing practice within the context of an integrated health facility, working in a unique and innovative clinical design model as part of a multidisciplinary sub specialty team, partnering with experts in the areas of fitness, nutrition, physical therapy, massage and psychology. The ideal candidate will possess a passion for outstanding patient care and thrive in a highly collaborative environment.

Duties and Responsibilities

• Assume quality of care for all patients, providing healthcare services that include but are not limited to comprehensive assessments, diagnosis, patient education, treatment, and referrals, as necessary.
• Provide comprehensive and ongoing care to a smaller patient panel, including in depth initial assessments, personalized health planning and high touch ongoing care. This includes a coaching-like approach to patient engagement.
• Assist Lead Physician in establishing medical policies, quality improvement processes, and evaluating current practices, policies and procedures.
• Adhere to the highest standards of medical practice, ethics, and professionalism at all times. Follow federal and state guidelines and statutes in the delivery of medical care services, including the prescription of medication.
• Contribute to open and honest ongoing communication with members and the team.
• Attend required meetings and participate in committees, as requested.
• Handle confidential matters concerning clinic policies and practices in accordance with HIPAA regulations.
• Maintain strict confidentiality.
